To make our job and your life a little bit more simple, please take the following steps when posting a bug report.
1) Please read the Documentation before posting.
2) Make sure you are using the latest version of UOX3 available in the Downloads section.
3) Ensure you have a relatively recent client version as well as the most up to date version of Razor, or whichever other tool you use to disable the client encryption.
4) Ensure that your problem is not on our Known Issues list, and that there are no other duplicate bug reports on the forum (just a brief glance at the post titles is fine).
5) Lastly, provide as much information as possible!

Your Razor Version.
Your UO Client Version.
Exactly what steps led up to your specific problem.
Have you re-started the server?
Has this happened multiple times / to multiple people?
Anything else of importance.
