Just to not leave this post unanswered: From the discussion me and Hoppertje had in IRC, and the testing based on that, it seems like using Razor with client versions above 6.0.10.x can cause these issues in UOX3 :/

For anyone else who runs into this issue, try disabling one or both of the following Razor-settings to see if it changes anything:
'Auto-Queue Object Delay actions'-setting
'Use Smart CPU usage reduction'-setting

Razor causes a lot of client problems because way the got it setup to hack the client to add there own features. Only thing the really update on razor is client support since the owner that made it disabandon the project to leave UO

i would say use client version 6.0.1.0 and up to 6.0.10.0 best versions out for uox3 at this moment.
